Security Fraud Litigation Appointments

Thank you for your interest in serving the State of Ohio as special counsel for securities fraud litigation. When filling out the forms below, please clearly communicate your firm’s experience, areas of expertise, and ability to perform the work requested.
 
The Attorney General’s Office requires completed RFQ forms to be submitted to SpecialCounsel@OhioAGO.gov by June 21, 2021.

  • Securities RFQ - This form is required by the Attorney General in order to review a firm’s qualifications.
  • Affidavit of Eligibility - This form is required by the Attorney General to ensure that a firm is in compliance with R.C. 3517.
  • Certifications - This form is required by the Attorney General to ensure that a firm and its attorneys are in good standing with the Ohio Bar and other state bars, are qualified under R.C. 3517.13, and have no legal malpractice actions that have been adversely decided against them within the last five years.
  • W-9 - This form is required by the Internal Revenue Service.
