VOCA/SVAA grant
This application is open to all applicants meeting eligibility guidelines for the respective grant programs. Decisions about grant awards will be determined through a discretionary evaluation process. Applications should be complete and in accordance with the criteria outlined in the program guidelines. No material, such as individual letters of support, articles of incorporation, etc., received after or apart from the application will be added to or considered with the application. Applications received with outdated or missing information may not be reviewed. Applicants must use the forms listed below. Do NOT create additional, replacement, or supplemental forms. Originals should be signed in blue ink.
All incomplete proposals will be considered late and all late proposals will receive a 1% decrease in the original allocation amount for every day incomplete or late.
Applications must be received by 5 p.m. on June 29, 2012.

- Programmatic Essay Questions
Upload answers to the 4 essay questions, no more than 500 words per response to each question.nbsp;
- Work Plan Essay - Example
The Work Plan essays explain the connections between the different components of the program or project. It is a useful tool for planning, implementation, and evaluation and for quickly explaining to others what your program is about. Responses to the Work Plan essays should take up no more than two pages for all of the questions together.
- Nonprofit organization information (If applicable)
Nonprofits must upload a document that includes the names and organizations of board members, plus a brief narrative on the board's operations that answers the following:
- What is the term of service for board members?
- What is the fundraising responsibility of the board?
- How frequently does the full board receive financial reports?
- How frequently does the full board meet?
- What, if any, long-range or strategic plan has the board adopted?
- What has the agency/board done in the last two years to enhance the organizational capacity of the agency?
- Coordinated Services Information
Upload contact information for one individual that your program works with from one of each of the four types of entities requested. These individuals will be contacted by the Attorney General’s Office during the grant period and asked to provide as assessment of your program’s service(s). (This process replaces the letters of support that have been required in previous years.)
- Nonprofit and public agency projected budget
Upload the organization's projected budget for the next fiscal year. Victim assistance budgets or victim-witness budgets should be clearly identifiable.
- Budget Revenue Information
Complete and upload the total dollar amount only of projected revenue to be received by your crime victims program for direct victims service by specific funding source.
- Budget narrative - Example
Prepare and upload a written statement for each cost category requested that gives complete budget detail and any supporting information that relates to the proposed costs. Include an explanation to clarify your calculations.
- Project match is required for all VOCA grants and a cash match is required for each VOCA-supported personnel position.
- SVAA grants do not require a project match.
- A position description is required for all VOCA and SVAA-supported personnel.
- Include worker's compensation information in budget calculations.
- Budget computations template (DOC)
Prepare and upload a computation for each cost category requested that gives complete financial detail and any supporting information that relates to the proposed cost category. Make sure to prioritize all computations as these computations represent your requests for specific funds. Applicants are responsible for assuring that figures are correct.

Prepare and upload personnel computations. The funding for a personnel position may be split between VOCA and SVAA funds. A separate computation needs to be completed for the VOCA portion of the page and the SVAA portion of the wage. A total wage rate is also requested.
