What is the Identity Theft Verification Passport Program?
The Passport Program allows victims of identity theft to verify to law enforcement and creditors that their identity has been stolen. Passport helps victims identify and correct fraudulent charges and repair their credit history.

How does an identity theft victim apply for and receive a Passport card?

What security measures are in place so that someone can’t get a Passport card and use it to impersonate the victim?
When creating a Passport application for a victim, law enforcement enters the victim’s photo, digital signature and fingerprint into the Ohio Law Enforcement Gateway, a secure Web site created by the Attorney General's Office for Ohio law enforcement to share information. These identifiers will be used to verify the identification of anyone claiming to be the victim. The card must be activated by the victim by calling a verification phone number from the phone number listed on the back of the Passport card. If an identity theft victim loses his or her Passport card, the Attorney Generals Office will deactivate the card and reissue a new card.
